Investigations into the OceanGate Titan submersible implosion revealed that the vessel had undergone 7 dives with a damaged hull before the fatal incident.

The United States National Transportation Safety Board (NTSB) found that the submersible's exterior featured "multiple anomalies" as early as 2022, including initial layered fractures known as delaminations in its carbon fiber composite exterior.

The submersible wasn’t damaged shortly before its final voyage, but initial layered fractures formed eight dives earlier, in July 2022.

The incident occurred in June 2023, while the vessel was en route to visit the wreckage of the RMS Titanic, resulting in the loss of all five passengers on board.
